Job summary

Meriden Health & Rehab is seeking a Licensed Social Worker to provide compassionate, compliant social services for residents in a skilled nursing setting. The role requires ensuring timely documentation and adherence to federal, state, and local guidelines while coordinating with families and staff.

You will lead discharge planning, collaborate with an interdisciplinary team, and support residents, families, and the care team daily.

Qualifications

• Experience as a social worker within a nursing home or rehabilitation center strongly preferred.
• Knowledgeable of the rules, regulations, and guidelines that govern nursing care facilities.
• Admission: 1 year (Preferred)
• Long term care: 2 years (Preferred)

Responsibilities

• Plan, organize, develop and implement all required social services to meet the needs of residents in accordance with federal, state and local guidelines that govern skilled nursing facilities.
• Performs all documentation requirements for social service documentation in a timely fashion.
• Completes all admission assessments and evaluations
• Works with the interdisciplinary team to create a plan of care for residents.
• Works with and provides support to residents, families and staff.
• Attends weekly Care Plan and Medicare meetings.
• Receives and attempts to resolve resident grievances.
• Coordinates resident discharge planning with all relevant departments and assists with scheduling outside services.
• Participates in monthly team meetings such as QA and Safety.
