Job summary

Seaside Skilled Rehabilitation Center is seeking a dedicated Social Worker to support residents' emotional and social needs and coordinate discharge planning in a long-term care setting.

The role requires a Bachelor’s degree in social work and a current Maine Social Work license, with long-term care experience preferred. You will collaborate with an interdisciplinary team to ensure compliant, person-centered care and family support.

Qualifications

• Bachelor's degree in social work and Maine Social Work license.
• Long-term care and skilled care experience preferred.
• Strong verbal and written communication; ability to work interdisciplinarily.
• All offers contingent on excellent references and Maine State Background Check; COVID-19 vaccination per Maine state requirement.

Responsibilities

• Lead on discharge planning, MDS completion, Level of Care and Rehab/UR meetings.
• Coordinate interdisciplinary care planning to meet resident needs.
• Assist in social services documentation and compliance with regulations.
• Support families and collaborate with nursing, therapies, and management.
