The Center for Living & Rehabilitation in Bennington, Vermont is seeking a compassionate Social Worker to join our subacute rehabilitation wing. You will provide emotional support and guidance to residents and their families, coordinating care with interdisciplinary teams.

Qualifications

• ASSOCIATES degree in Social Work or related field.
• 1 year of experience in social work.
• Licensure as a Social Worker in the state of Vermont.

Responsibilities

• Provide emotional support and guidance to residents and their families, addressing their mental health and psychosocial needs
• Conduct psychosocial assessments and develop individualized care plans to meet residents' social and emotional needs
• Facilitate communication between residents, families, and healthcare teams to ensure comprehensive care
• Develop and implement interventions to address residents' mental health and cognitive issues
• Collaborate with interdisciplinary teams to ensure resident-centered care and effective discharge planning
• Maintain accurate records and documentation of social work services and interventions
