Bayview Rehabilitation at Scalabrini and Healthcare Center, a 120-bed sub-acute and long-term care facility in North Kingstown, RI, is seeking a full-time Social Worker. You will assess psychosocial needs, develop care plans, and coordinate services to support residents and families.

The ideal candidate holds a Bachelor's degree (Master's preferred) with 1+ years in healthcare social work, strong communication, and ability to thrive in a team. Daily Pay is available for this role.

Qualifications

• Bachelor’s degree required; master’s degree strongly preferred.
• Minimum 1+ years of experience as a Social Worker in healthcare setting.
• Strong knowledge of regulations governing nursing facilities.
• Excellent communication and interpersonal skills.
• Ability to work in a fast-paced, team-oriented healthcare environment.

Responsibilities

• Evaluate residents to assess psychosocial needs, strengths, and limitations.
• Address resident concerns and emotional needs with ongoing communication to residents, families, and case managers.
• Build and maintain positive relationships with residents and families.
• Coordinate care and collaborate with interdisciplinary teams.
• Develop and update individualized psychosocial care plans.
• Participate in care planning meetings and support continuity of care.
• Provide counseling and resource referrals as needed.
• Ensure compliance with facility policies and documentation standards.
