Job summary

East Side House seeks a Social Worker to support Expectant Families and Older Adult Programs in multisite locations. The role involves assessments, counseling, crisis support, referrals, and cooperation with program staff to help families stabilize and access needed services.

Qualified candidates have an MSW (licensure not required), 2–3 years of related experience, and strong English/Spanish communication.

Qualifications

• Master of Social Work (MSW); social work licensure not required.
• Minimum of 2-3 years of relevant experience in social services, case management, family support, community-based services, or related setting.
• Bilingual English/Spanish strongly preferred.
• Experience with expectant families, families with young children, older adults, or diverse populations preferred.
• Knowledge of NYC public benefits, emergency assistance, healthcare, behavioral health, housing, food security, and related resources.
• Strong written and verbal communication skills; proficient with electronic case-management/data systems and Microsoft Office.

Responsibilities

• Conduct initial and ongoing needs assessments and develop support referral and follow-up plans.
• Coordinate emergency assistance and basic-needs support, including required documentation.
• Provide short-term supportive counseling, crises support, safety planning, and referrals.
• Connect participants to internal and community resources and public benefits.
• Plan and facilitate psychoeducational workshops, groups, and events based on participant needs.
• Collaborate with staff to identify participant needs and coordinate services.
• Maintain timely, accurate documentation of assessments, services, referrals, and outcomes.
• Fulfill mandated reporter responsibilities and respond to safety concerns.
