Job summary

AuthoraCare Collective is seeking a Weekend (Home Care & Long-term Care) Social Worker for the Greensboro service areas with flexibility to support Burlington as needed. This is a part-time role, every other Saturday and Sunday 8:00am - 7:00pm.

The role requires a Bachelor's or Master’s degree in social work and two years of social work experience in a mental health or medical setting, plus CPR certification.

• Two years of experience in social work in a mental health or medical setting.

Responsibilities

• Provide intervention and assistance to the Social Work Department.
• Conduct resource assessment and planning, intervention, evaluation, and case management.

• Travel to and work in patients' homes within the service area.

AuthoraCare Collective is currently seeking a Weekend (Home Care & Long-term Care) Social Worker for our Greensboro service areas with flexibility to support our Burlington service areas as needed. This is a part-time position; every other Saturday and Sunday 8:00am - 7:00pm.

Job Summary

Responsible for providing intervention and assistance to the Social Work Department which is consistent with basic generalist social work practice. This includes: resource assessment and planning, intervention, evaluation, case management, information and referral, supportive presence/supportive counseling, case management, advocacy, community resource and client. Also included is long-term care planning to consist of LTC placement, completion of appropriate documentation i.e. FL-2s and obtaining PASRRs. Must be able to travel to, enter, and function in the homes of any patient/caregiver within the service area.

Requirements

Education/Qualifications:

• Bachelor's degree or Master's degree in social work required.
• Two years experience in social work in a mental health or medical setting is required.
• CPR certification from the American Heart Association or American Red Cross Basic Life Support (BLS) is required.
• Must be able to travel to, enter, and function in the homes of any patient/caregiver within the service area.
• Ability to concentrate with large volumes of work, handle pressure of deadlines, good judgment, ability to follow procedures, ability to work independently and with frequent interruptions.
• Communicate effectively with persons having highly diverse backgrounds and educational levels.
• Work effectively as a member of the interdisciplinary team.
• Other: Valid state-issued driver's license required. Must carry automobile liability insurance at limits required by agency. Must have own transportation.
