Job summary

AuthoraCare Collective is seeking a Hospice RN for our After-hours department, Home Care/Long Term Care (End of Life Visits) serving the Burlington area. This is a part-time role with coverage every other Saturday and Sunday from 8:00am to 2:00pm.

The RN will assess patient/caregiver needs, develop care goals, document outcomes, and collaborate with the medical team to implement plans of care. Strong communication with patients, families, and providers is essential.

Qualifications

• Graduate of an accredited School of Nursing.
• Must be currently licensed as an RN in North Carolina and in good standing with the NC BON.
• One year of experience in an acute, hospice, home health or long-term care setting.

Responsibilities

• Assess and identify patient/caregiver problems and establish care goals.
• Identify and implement interventions for agency programs and document outcomes.
• Collaborate with physicians to establish and implement medical care plans.

AuthoraCare Collective is currently seeking a Hospice RN for our After-hours department, Home Care/Long Term Care (End of Life Visits) supporting our Burlington service area. This position is Part-time; Every other Saturday and Sunday 8:00am - 2:00pm.

Job Summary:

Assess and identify patient/caregiver problems, establish nursing patient/caregiver care goals, and identify and implement interventions for all agency programs. Will document outcomes of care. Work with physician to establish and implement medical plan of care for the patient. Work closely with other members of the interdisciplinary group and communicate effectively both verbally and in writing with group members, patients, caregivers, physicians, pharmacists, and other health care professionals and community service organizations. Establish and maintain therapeutic relationship with patient/caregiver to include providing emotional support and teaching.

Requirements

Education and Experience :

• Graduate of an accredited School of Nursing
• Must be currently licensed as an RN in North Carolina and in good standings with NCBON.
• One year of experience in an acute, hospice, home health setting, or long-term care setting plus.

Other: Valid state-issued driver's license required. Must carry automobile liability insurance at limits required by agency. Must have own transportation.
