Job summary

Westhab is hiring a Client Care Coordinator (LMSW) for our Manhattan/Hudson-41st Street Shelter in New York. The role is full-time, non-exempt, based on-site in NYC with a salary range of $58,000–$60,000 per year.

The LMSW will coordinate mental health and substance abuse services for high-risk families and manage a small caseload while providing crisis intervention as needed. The qualified candidate has an MSW (CSWE-accredited), at least two years in a related field, and Spanish language skills

Qualifications

• LMSW required; master's degree in social work from a CSWE-accredited program.
• At least two years of experience in a specialized social work area.
• Fluency in Spanish is highly desirable.

Responsibilities

• Coordinate outpatient and inpatient mental health and substance abuse services for families with children in a homelessness shelter.
• Facilitate weekly mental health and substance abuse workshops and onsite support groups.
• Identify community resources and make referrals to services and governmental agencies.
• Independently assess psychosocial functioning and needs to formulate and implement a treatment plan.
• Conduct psychosocial assessments from a strength-based, client-centered perspective.
• Provide consultation to staff on psychosocial needs and impact on health care and treatment adherence.
• Maintain and sign off on documentation and reports in a timely manner.
• Identify program needs and make recommendations.
• Perform related tasks as assigned.
