Job Summary
As a Social Worker with Northfield Hospital + Clinics, you will provide services to address the needs of patients and family members through psychosocial assessment, discharge planning, emotional support, and resource provision.

Compensation
The hourly wage range for this position is: BSW $28.28-$39.92; MSW $31.81-$44.90, based on experience, education, and internal equity.

Schedule
This is a casual/call position with no regular hours, covering shifts 8:00am-4:30pm, Monday through Friday, as needed.

Key Accountabilities

1. Provide psychosocial assessments for patients and families across all hospital units, including medical/surgical, emergency, birth center, and clinic.
2. Offer social work services such as supportive counseling, discharge planning, and community resource referrals.
3. Work effectively with clients of all ages and cultural backgrounds.
4. Maintain knowledge of local referral resources.
5. Collaborate with the healthcare team to develop care plans.
6. Perform other duties as assigned in a fast-paced environment.

Qualifications

• Licensed by the Minnesota State Board of Social Work; Master's level preferred.
• At least 2 years of healthcare social work experience preferred, including hospital or nursing home settings.
