Job Description Summary:

This position ensures organizational excellence related to customer service and quality of care while promoting excellence in the cultural experience at the assigned hospital. Responsibilities include providing biopsychosocial assessments, psychiatric diagnoses, treatment planning and interventions, therapy sessions (individual, couple, family, or group), and discharge planning to support biopsychosocial functioning. The role involves receiving referrals from interdisciplinary team members for individuals from at-risk populations.

Must demonstrate knowledge and skills necessary to provide appropriate care, including understanding growth and development across life cycles. The ability to act quickly in high-stress situations is required.

Responsibilities And Duties:
  1. Clinical Assessment (30%): Conduct interviews, chart reviews, and collaboration to interpret biopsychosocial functioning; develop care and discharge plans; identify cases of abuse or neglect; provide crisis intervention and community resource linkage.
  2. Intervention Management (30%): Develop intervention plans; employ clinical skills including counseling, crisis intervention, and educational planning.
  3. Population Management (25%): Coordinate clinical and financial outcomes; collaborate with staff and external organizations; monitor service quality; liaise with payers; ensure treatment plan completeness.
  4. Documentation and Regulatory Compliance (10%): Document assessments and interventions accurately; comply with documentation standards and regulations.
  5. Additional Functions (5%): Maintain awareness of community resources; facilitate service linkages; perform other duties as appropriate.
Minimum Qualifications:

Master's Degree in Social Work; Ohio LSW license; Driver's License if in a Home Care department; meet specific licensure requirements.

Part-Time Casual Role:

Supports Virtual Care Management team for evening, weekend, and holiday coverage. Candidates should possess relevant knowledge and skills listed below.

SPECIALIZED KNOWLEDGE
  • Master's in Social Work or related field and Ohio LSW license.
  • 1-2 years’ experience as an LSW, familiarity with medical terminology, computer skills.
  • Grandfathered BSW license holders prior to 01/10/2019; others must meet current qualifications.
  • Interview, assessment, organizational, and problem-solving skills.
  • Ability to identify community resources and collaborate effectively.
  • Patience and tactful communication skills.
  • Commitment to quality patient care, customer service, safety, and cost efficiency.
