Job DescriptionThe Case Manager reflects the mission, vision, and values of NM, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines, and all other regulatory and accreditation standards.
The Case Manager serves as the primary liaison between the interdisciplinary team and the patient/caregiver, educates the patient/caregiver on the continuum of care, and facilitates discharge coordination. Follows the patients from admission to discharge. The case manager communicates to external payers on behalf of the patient. Acts in partnership with the attending physician to closely monitor utilization and LOS. Works with the patient/caregiver to plan and facilitate a timely and appropriate discharge plan.
Responsibilities:Master’s degree in social work with current LSW or LCSW license in the state of Illinois.
Preferred:Previous experience with Case Management and/or Discharge Planning in a hospital setting
Previous experience in a rehabilitation setting
