Job summary

This role collaborates with interdisciplinary teams to ensure safe transitions of care, advocates for patient needs, and connects patients to community resources; requires MSW and active Ohio licensure; BLS within 90 days.

Qualifications

• Master's degree in Social Work (MSW).
• At least 1 year of clinical social work experience in a healthcare setting.
• Current Ohio LSW or LISW licensure; must maintain active licensure per state law.
• Basic Life Support (BLS) certification, or ability to obtain within 90 days of hire.

Responsibilities

• Conduct comprehensive psychosocial assessments and provide counseling and support.
• Coordinate care plans to address social, emotional, and resource needs of patients and families.
• Collaborate with interdisciplinary teams to facilitate safe transitions of care.
• Advocate for patient needs and connect patients to community resources.
