Job summary

Qualifications

• Master's degree in Social Work (MSW).
• At least 1 year of clinical social work experience in a healthcare setting.
• Current Ohio Licensed Social Worker (LSW) or Licensed Independent Social Worker (LISW); must maintain active licensure in accordance with Ohio state law.
• Basic Life Support (BLS) certification, or ability to obtain within 90 days of hire.

Responsibilities

• Conducts comprehensive psychosocial assessments, provides counseling and support, and coordinates care plans to address social, emotional, and resource needs of patients and families.
• Collaborates with interdisciplinary teams to facilitate safe transitions of care.
• Advocates for patient needs and connects patients to community resources.
• Performs other duties as assigned in alignment with Kettering Health's mission, vision, values, and B Safe program.
