Qualifikationen

• Minimum 1+ year of healthcare social work experience; nursing home/rehabilitation center preferred.

Aufgaben

• Evaluate residents to assess psychosocial needs, strengths, and limitations.
• Address resident concerns and goals while communicating with residents, families, and case managers.
• Build and maintain positive relationships with residents and families.
• Coordinate care with interdisciplinary teams including physical therapy and other clinical departments.
• Develop, implement, and update individualized psychosocial care plans.
• Participate in care planning meetings and support continuity of care.
• Provide counseling, emotional support, and resource referrals as needed.
• Ensure compliance with facility policies, regulations, and documentation standards.
