Bi-Lingual Social Worker/Care Manager

JOB DESCRIPTION

Jewish Family and Children’s Service of Greater Philadelphia has a unique and vital mission: to strengthen families and individuals across generations and cultures to achieve stability, independence and community

The Bi-Lingual Russian Speaking Care Manager will provide comprehensive case management services to Holocaust Survivors and their families, with a particular focus on supporting Russian-speaking clients. This role ensures that Survivors receive compassionate, culturally sensitive care, advocacy, and access to services that promote dignity, independence, and quality of life. The Care Manager will conduct assessments, develop individualized care plans, and coordinate community-based resources while maintaining compliance with agency, funder, and regulatory standards.

RESPONSIBILITIES
  • Provides Russian/English care management including:
  • Program eligibility screening;
  • Assistance to Care Navigator with Russian intake calls and wait list clients;
  • Assessments and documentation of clients' functionality and service need;
  • Assists clients with enrolling in/applying for benefits and services that will help clients continue to live safely at home;
  • Facilitation of department programming in conjunction with collaborating departments at JFCS
  • Participates in one-on-one supervision with Director, departmental meetings and trainings;
  • Engages in networking and collaboration with community-based organizations and resources on behalf of clients;
  • Performs other duties as assigned by immediate supervisor as requested.
  • Participates in team conferences and peer supervisory sessions
  • Is culturally competent to serve populations with varying backgrounds
  • Performs other duties as assigned by immediate supervisor and other management as requested
REQUIREMENTS
  • Bachelor's degree in a social service field required, Master's degree preferred
  • Fluency in Russian and English required, reading, writing, speaking
  • Ability to demonstrate strong working knowledge of community resources and benefits relevant to older adults and their family caregivers
  • Proficient user of technology (smart phone/tablet/laptop) and experience using MS Office applications required
  • EHR/EMR database experience a plus

  • Must be flexible and agile to be able to juggle tasks when special situations arise that demand attention
OTHER REQUIREMENTS
  • Employee must have a safe driving record on file with the Department of Motor Vehicles in the state in which the employee is licensed to drive and provide a valid driver's license and proof of automobile liability insurance at time of employment. Employee must maintain a valid driver's license and carry liability insurance as a condition of continued employment.
  • May be asked to work flexible hours, (one evening per week) to match the availability of individuals and families
  • May participate in group programming that runs beyond normal business hours (schedule will be adjusted accordingly).
  • All prospective employees must pass a criminal background check, child abuse check and FBI fingerprint clearances
