Job summary

The Holocaust Survivor Homecare Case Worker at JCCGCI Inc. provides compassionate, client-centered support to Holocaust Survivors enrolled in HSSS Programs in Brooklyn, NY.

You will manage a Homecare Case Work caseload, conduct home visits, and develop individualized care plans with attention to social, emotional, and health needs. Requires strong organizational and communication skills, empathy, and the ability to collaborate with healthcare providers and community resources.

Qualifications

  • High school education required; college degree in social work (BSW) or related field preferred.
  • Experience with older adults, trauma survivors, or vulnerable populations is highly desirable.
  • Strong written and verbal communication; culturally sensitive.
  • Bilingual Yiddish and/or Russian preferred.

Responsibilities

  • Maintain a Homecare Case Work caseload.
  • Conduct comprehensive home visits to intake and evaluate clients’ physical, emotional, and social needs.
  • Identify unmet needs of each Homecare client and coordinate services.
  • Develop individualized care plans tailored to each survivor’s circumstances.
  • Maintain communication with clients and families through calls and visits.
  • Collaborate with healthcare providers and community resources to ensure quality care.
  • Arrange homecare services, counseling, financial assistance, and other support.
  • Advocate for clients to ensure respectful, comprehensive care.
  • Maintain accurate client records and prepare reports on status and needs.
  • Participate in staff meetings and trainings; perform additional duties as assigned.

Description

The Holocaust Survivor Homecare Case Worker plays a vital role in providing compassionate, comprehensive support to Holocaust Survivors enrolled in JCCGCI’s HSSS Programs. This position requires empathy, strong organizational skills, and a commitment to improving the quality of life for Holocaust Survivors.

Job Responsibilities
  • Maintain a Homecare Case Work caseload.
  • Conduct comprehensive home visits to intake and evaluate clients’ physical, emotional, and social needs.
  • Identifying unmet needs of each Homecare client (in addition to Homecare, such as health, social, mental health, financial, quality of life and other needs).
  • Develop individualized care plans tailored to each survivor’s circumstances and preferences.
  • Maintain consistent communication with clients and their families through regular check-in calls and in-person visits.
  • Collaborate with healthcare providers, community agencies, and other support services to ensure clients receive quality care and appropriate resources.
  • Assist in arranging homecare services, counseling, financial assistance, and other vital support systems.
  • Advocate on behalf of clients to ensure they receive respectful, comprehensive, and compassionate care.
  • Maintain accurate, timely, and complete client service records in accordance with funder, regulatory, and agency requirements.
  • Prepare regular reports summarizing client status, service delivery, and any ongoing needs or concerns.
  • Participate in staff meetings and trainings as required.
  • Additional duties, as assigned.
Job Requirements
  • High school education required; college degree in social work (BSW) or a related field or equivalent experience in social services or case management preferred.
  • Experience working with older adults, trauma survivors, or vulnerable populations is highlydesirable.
  • Strong written and verbal interpersonal and communication skills, with empathy and culturalsensitivity.
  • Ability to manage multiple cases, and maintain organized, timelydocumentation.
  • Ability to prioritize tasks and manage time effectively.
  • Capacity to work both independently and collaboratively in a fast-paced and dynamic environment.
  • Knowledge of local social services, healthcare systems, and communityresources.
  • Proficiency in Microsoft Word, Excel, and data entry systems.
  • Bilingual Yiddish and/or Russian preferred.

Pay: $27.00 - $28.00 per hour.

Schedule: 35 hours per week.

This role entails In-Office work with travel to clients' homes required.
