Job summary

Your Health Organization in the Columbia area is seeking a qualified Social Worker to assist patients across facilities and homes, performing advance care planning, assessments, and care coordination.

This full-time role requires reliable transportation and daily travel with 8am-5pm shifts Monday to Friday. The position covers Columbia, Blythewood, Winnsboro, and surrounding areas, offering a comprehensive benefits package and opportunities for growth.

Qualifications

• Qualified Social Worker with patient-facing experience.
• Strong communication and interpersonal skills.
• Ability to coordinate care across facilities and homes.

Responsibilities

• Initiate Advance Care Planning for patients throughout their assigned Care Team (including facilities).
• Complete Daily Comprehensive Assessments on patients.
• Create Daily Care Plans for patients.
• Document Chronic Care Management time daily.
• Meet Daily Productivity Goals.
• Coordinate discharges at contracted facilities.
• Coordinate Placement.
• Maintain continuity of coverage for acute visits and changes in med.
• Build and maintain relationships with referral sources and Care Teams.
