Job summary

Your Health Organization in Columbia area is seeking a qualified Social Worker to assist patients across facilities and homes. This full-time role involves home and facility visits in the designated service area, with daily travel and 8-hour shifts from 8am to 5pm.

The Social Worker will support advance care planning, assessments, care plans, and coordination with care teams. The position requires compassionate service, strong communication, and the ability to build relationships with referral

Qualifications

• Qualified Social Worker to assist patients across facilities and homes.
• Excellent customer service skills expected to support patients and families.
• Ability to coordinate care and communicate with care teams.

Responsibilities

• Initiating Advance Care Planning for patients throughout assigned Care Team (including facilities).
• Completing Daily Comprehensive Assessments on patients.
• Completing Daily Care Plans on patients.
• Documenting Chronic Care Management time daily.
• Meeting Daily Productivity Goals.
• Coordinating discharges at contracted facilities.
• Coordinating Placement.
• Continuation of coverage (acute visits for falls, med changes, etc.).
• Build and maintain professional relationships with referral sources.
• Maintain strong communication with patients, families, and Care Teams.
