Job summary

Your Health Organization in Columbia area is seeking a Social Worker to support patients across multiple facilities and homes. The role involves home and facility visits, coordinating placements, and managing care plans.

Daily visits, travel is required, and the position is full-time, typically 8-hour shifts, Monday through Friday. The Social Worker will initiate advance care planning, perform comprehensive assessments, and collaborate with care teams to deliver compassionate, coordinated care

Responsibilities

• Initiate Advance Care Planning for patients across assigned Care Team and facilities.

• Meet Daily Productivity goals.
• Coordinate discharges at contracted facilities.
• Arrange placement as needed.
• Ensure continuity of care during acute visits or changes in meds.
• Build and maintain professional relationships with referral sources.
• Maintain clear communication with patients, families, and Care Teams.

Social Worker - Support Service Coordinator (team1 and2)

We are seeking a qualified Social Worker to assist our patients in our Columbia area. This candidate should be a compassionate Social Worker with excellent customer service skills to assist our patients' multiple facilities and homes in the area.

This role will perform visits in homes and facilities (ALF and ILF's) in their designated service area. You must have reliable transportation as travel is required daily. This is a full time, salary-based working 8-hr shifts Monday-Friday (8am-5pm)

What Are We Looking For?

Your Health is currently looking for a Social Worker to join our growing urgent care family. This role would be working with patients to help complete their advance care planning, cognitive assessments, placement, general resources, etc. The Social Worker would be responsible for the following:

• Initiating Advance Care Planning for patients throughout their assigned Care Team (including facilities).
• Completing Daily Comprehensive Assessments on patients.
• Completing Daily Care Plans on patients.
• Documenting Chronic Care Management time daily.
• Meeting Daily Productivity Goals
• Coordinating discharges at contracted facilities
• Coordinating Placement.
• Continuation of coverage (i.e. coordinating acute visits for falls, med changes, etc.)
• Build and maintain professional relationships with referral sources.
• Maintain strong communication and professionalism between referral sources, patients/families, and Care Teams.
