Job summary

Southern California Hospitals is seeking a Social Worker to deliver psychosocial evaluation and discharge planning, advocate for patients, and refer to community services, while ensuring clear documentation in the medical record.

You will interview patients and families, collaborate with physicians and staff to understand medical conditions, and make recommendations for additional services at discharge. Strong English communication and teamwork are required.

Qualifications

• BSW or related degree in human services.
• Strong written and verbal communication skills in English.
• Knowledge of CMS, Medicare/Medi-Cal.
• Familiarity with Joint Commission, CMS, CDPH requirements.
• Ability to establish and maintain effective working relationships across the organization.
• Ability to facilitate and lead interdisciplinary rounds.

Responsibilities

• Interviews patients and significant others to obtain information about social, emotional, and family situation; communicates information to physicians and care team.
• Participates in conferences with patients and healthcare staff to arrange services with community agencies.
• Provides post-hospital plan referrals and arrangements, ensuring HIPAA compliance.
• Demonstrates empathy and culturally appropriate care in interactions with patients and families.
• Contributes to departmental studies and reports as required by Social Service Department.
