Job summary

Advocate Health is seeking a licensed social worker to join our multidisciplinary hospital team. You will conduct psychosocial assessments, develop care plans, and coordinate discharge planning from admission through transition, engaging patients and families in decision-making.

You will partner with nursing, physicians, case managers, and community resources to ensure timely, appropriate care routes, comply with CMS regulations, and support efforts to reduce readmissions while upholding NASW

Qualifications

• IL-LSW eligible; LCSW preferred per Illinois requirements.
• WI Social Work Certification issued by Wisconsin.
• MSW required; CSWE accreditation preferred; NASW ethics adherence.
• GA certification preferred; NASW Code of Ethics compliance.
• Two years of healthcare social work experience.

Responsibilities

• Conduct psychosocial assessments, develops care plans, initiates interventions with patient/family and care team.
• Provides social work services including screening, needs evaluation, interventions and discharge planning.
• Participates on the multi-disciplinary team for post-acute placement and community resources; Advocates for patient involvement.
• Completes UM activities, documents CMS-required updates, and coordinates denial activity when needed.
• Communicates with the healthcare team and coordinates barriers to discharge with MDR and medical staff.
• Maintains knowledge of community resources, aging and protective services, and supports post-discharge connections.
• Educates patients and families about resources, advance directives, and end-of-life matters.
• Collaborates with community agencies to plan continued care and reduce readmissions.
