Qualifications

• MSW required.
• 1–2 years' experience in frontline social services.
• Experience in ambulatory health center preferred.

Responsabilités

• Provide frontline social services to patients and families.
• Coordinate psychosocial community services per policies and protocols.
• Act as Community Outreach liaison for Priority Care and Special Needs Population.
• Assist with placements and paperwork (DMVs, long-term care, disability, transportation, insurance).
• Maintain statistical data and provide written reports for the program.
• Educate patients and families about health care teams, entitlements, and resources.
• Coordinate patient discharge and continuity of care planning.
• Document all patient communications in charting notes.
