• Complete comprehensive psychosocial assessments and develop member-specific actions.
• Collaborate with members, families, and healthcare providers.
• Identify referrals to financial, psychosocial, community and state services.
• Advocate for members to navigate the healthcare system.
• Maintain records of plans, interventions and outcomes.
