Qualifikationen

• Master’s or Doctoral degree in Psychology, Social Work, Counseling, MFT, or related field.
• Active, unrestricted NY license (LMHC/LMFT/LCSW/LP) required.
• 2+ years of experience in a related mental health environment.
• Proficient in MS Office; working-from-home setup.

Aufgaben

• Identify and provide community service linkage.
• Assist enrollees with aftercare appointments.
• Provide follow-up to ensure continued access to care.
• Collaborate with providers to develop aftercare plans.
• Meet with community providers to build partnerships.
