Kaleida Health is seeking a Social Worker in the HPTE SNF Social Work department in Buffalo, NY. The role involves assessing the psychosocial needs of long-term care residents and coordinating admission, discharge planning, and financial support with the care team.

Required is a BA in Social Work (or related field) and NYS driver's license. At least two years in long-term care or medical discharge planning is expected, with MDS proficiency preferred.

Job Description

Assess the psychosocial needs of long term care residents and provide support services (admission, discharge planning, financial, insurance and MDS) in accordance with professional social work standards and in collaboration with the interdisciplinary health care team.

Education And Credentials

BA - Bachelor of Arts or Bachelor of Science in Social Work degree or related field Required. NYS Drivers License upon hire required.

Experience

2 years of experience in Long term care social work or medical in-patient discharge planning required. Professional experience in Long term care, acute care, MRU or subacute rehab required. Computers, faxes, routine office machinery preferred. Proficiency with MDS Preferred.
