SUMMARY: This position is responsible for the delivery of social work services to identified home care clients. The Social Worker will provide psychosocial support and counseling to referred clients and will facilitate financial and long-term care planning as needed. The Social Worker is responsible for communicating with health care team and documenting all care activities. The incumbent manages multifaceted complex family dynamic cases.

$61,360.00 - $79,040.00 CITY: Lakeville POSTAL CODE: 14480 The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
