Kaleida Health is seeking a Social Worker in the HPTE SNF Social Work department in Buffalo, NY. The role involves assessing the psychosocial needs of long-term care residents and coordinating admission, discharge planning, and financial support with the care team.

Required is a BA in Social Work (or related field) and NYS driver's license. At least two years in long-term care or medical discharge planning is expected, with MDS proficiency preferred.

Qualifications

• BA in Social Work or related field required.
• NYS Driver's License required upon hire.
• 2 years of experience in long-term care social work or medical inpatient discharge planning required.
• Experience in long-term care, acute care, MRU or subacute rehab preferred.
• Proficiency with MDS preferred.

Responsabilités

• Assess the psychosocial needs of long-term care residents and provide support services in accordance with professional social work standards.
• Collaborate with the interdisciplinary health care team on admission, discharge planning, financial and insurance matters.
