Job summary

CommonSpirit Health is seeking a Social Worker to advocate for individuals and families facing complex challenges. The role includes psychosocial assessments, care planning, counseling, crisis intervention, and connecting clients with resources.

You will collaborate with multidisciplinary teams to ensure holistic support, maintain records, and advocate for clients in the community. Sign-on bonus available for eligible hires.

Qualifications

• Bachelors in Social Work is required.
• Licensed Social Worker in North Dakota (ND).
• One year of healthcare experience required.
• Driver's License: ND is required.
• Auto insurance is required for the role.
• Basic Life Support (CPR) certification required.

Responsibilities

• Assess the psychosocial status of clients related to illness and environment and communicate findings to the RN.
• Carry out social evaluations and plan interventions based on evaluation findings.
• Maintain clinical records on all clients referred to social work.
• Provide information and referral services to clients and families/caregivers regarding environmental needs.
• Provide information to clients or families/caregivers and community agencies.
• Market and promote agency programs and events in the local community.

Every day, you will conduct psychosocial assessments, develop care plans, provide counseling and crisis intervention, and connect clients to resources. You'll advocate for clients and collaborate with multidisciplinary teams to ensure holistic support and promote well-being.

To be successful in this role, you will possess exceptional interpersonal skills, empathy, strong knowledge of community resources, and proven crisis management abilities, fostering positive change and client empowerment.

Pay Range

$28.80 - $42.84 /hour
