CommonSpirit Health at Home is seeking a Medical Social Worker for a PRN position serving Monument, Colorado Springs, Fountain and Manitou Springs. You will collaborate with physicians to plan care, provide crisis intervention, and support patients with social, economic and emotional needs within a home-health setting.

Qualified candidates hold a CSWE-accredited MSW, state licensure, CPR/BLS certification, and at least one year of health-care social work experience.

Qualifications

• Master’s degree from a CSWE-accredited program.
• Current state license to practice social work.
• Minimum 1 year social work experience in healthcare.
• CPR/BLS certification (AHA or Red Cross).

Responsabilités

• Create a care plan aligned with the physician’s treatment plan.
• Provide crisis intervention as needed.
• Assist physicians and caregivers with social, emotional and economic factors affecting medical progress.
• Educate patients and caregivers on care plans and resources.
• Refer to community resources and public assistance when appropriate.
• Collaborate with caregivers to maintain optimal patient-caregiver relationships.
• Complete and submit documentation accurately and timely.

Job Summary And Responsibilities

This is a PRN position and will be primarily for service areas of Monument, Colorado Springs, Fountain and Manitou Springs

Responsibilities

At CommonSpirit Health at Home, we strive to embody our mission of delivering hope and healing to those we serve. As a Medical Social Worker, you’ll engage in a range of exciting responsibilities, including:

• Establishing a care plan in accordance with the physician’s plan of treatment.
• Providing crisis intervention as required.
• Assisting the physician and other caregivers in understanding the significant social, emotional and economic factors influencing the patient’s ability to improve medically.
• Providing patient care education to patients and caregivers.
• Assisting in making appropriate community referrals to gain access to additional resources as needed (i.e. applications for public assistance, housing problems, additional health care resources, transportation)
• Working with the patient’s caregivers to resolve problems interfering with an optimum caring relationship between the patient and caregiver.
• Completing and submitting all required documentation in an accurate and timely manner.

Job Requirements

• Master’s degree from an accredited school approved by the Council on Social Work Education.
• Currently licensed to practice in the state serviced.
• One year social work experience in a health care setting.
• Current CPR/BLS certification through American Heart Association or American Red Cross and compliance with state requirements.

Pay Range

$28.10 - $46.37 /hour
