Job summary

CommonSpirit Health in Dickinson, ND is seeking a compassionate Social Worker to advocate for individuals and families facing complex challenges. You will conduct psychosocial assessments, develop care plans, provide counseling and crisis intervention, and connect clients to resources to promote well-being.

You will collaborate with multidisciplinary teams, maintain clinical records, and advocate for clients, aiming to empower communities and improve health outcomes across our care sites.

Qualifications

• Bachelor's degree in social work required
• ND licensed social worker required
• Minimum 1 year healthcare experience
• Driver's license: ND

Responsibilities

• Assesses the psychosocial status of clients related to the client’s illness and environment and communicates findings to the registered nurse
• Carries out social evaluations and plans intervention based on evaluation findings
• Maintains clinical records on all clients referred to social work
• Provides information and referral services to clients and families/caregivers regarding environmental needs
• Provides information to clients or families/caregivers and community agencies
• Markets and promotes the agency programs and events in the local community

Every day, you will conduct psychosocial assessments, develop care plans, provide counseling and crisis intervention, and connect clients to resources. You'll advocate for clients and collaborate with multidisciplinary teams to ensure holistic support and promote well-being.
