Job summary

Emplify Health by Gundersen Health System is hiring a full-time Behavioral Health Social Worker in Onalaska, WI. You will provide assessment, planning, education and referral services to Emplify Health patients and family members and act as a consultant and leader in comprehensive care across the system.

The role requires a Bachelor's degree in Social Work (or related field), a WI Social Worker license, and at least 1 year of direct patient contact with crisis intervention experience.

• 1 year of experience with direct patient contact. Interviewing/supportive counseling experience. Crisis Intervention experience.

Emplify Health by Gundersen Health System is now hiring a full-time Behavioral Health Social Worker in Onalaska! Social Workers provide assessment, planning, consulting, education and referral services to Emplify Health patients and family members as well as act as consultants and leaders in comprehensive care throughout the system, while supporting organizational strategies and goals.

What You Will Work

• 1.0 FTE = 80 hours every 2 weeks
• Monday – Friday, core business hours. No weekends or holidays.
• Location: Onalaska, WI

What You Will Do

• Social Worker in the department of Behavioral Health is responsible for any of the following:
  • Assess incoming referrals, schedule appointments, coordinate patient care, provide or assist with crisis intervention, meet with patients, facilitate/co-facilitate groups, and assist with programming needs.
