Job summary

Luther Manor is seeking a dedicated Social Worker to support residents' emotional and psychosocial well-being through assessments, care planning, advocacy, and collaboration with families and care teams. You will ensure person-centered care and timely documentation.

You will assess needs at admission and changes in condition, develop and update care plans, coordinate services, and act as a liaison among residents, families, and the interdisciplinary team while honoring each resident's dignity

Qualifications

  • Bachelor's degree in Social Work or related behavioral science.
  • Three to five years of experience in Long-Term Care, Assisted Living or Community Based Facilities.
  • Ability to document resident care plans concisely and clearly.
  • Must be available for on-Call for weekend concerns or resident admission on a rotation basis.
  • Attend in-services/classes concerning social workers field and to maintain social work certification.

Responsibilities

  • Identify residents' psychosocial, mental and emotional needs and facilitate access to services to meet those needs.
  • Plan discharge as appropriate; arrange home care services and assist with transfer to another location.
  • Act as a liaison between resident, family and interdisciplinary team.
  • Document all resident care plan information and adhere to State and Federal codes.
  • Advocate for residents and participate in solutions to identified problems.
  • Collaborate with other departments to evaluate residents' psychosocial wellbeing, mood, and behavior concerns.
  • Refer residents/families to community resources for concerns in finance, housing and services.
  • Take leadership role in investigation of resident/family concerns, follow through to conflict resolution and reporting to authorities as necessary.
  • Perform assessments at admission, upon change in condition and annually to identify needs.
  • Create, review, and update plan of care to support individual needs.
  • Promote actions by staff that maintain or enhance each resident's dignity.

Education

Bachelor's degree in Social Work or related behavioral science

Summary

Qualifications
  • Bachelor's degree in Social Work or other related behavioral science. Social Work certificate for state of Wisconsin, preferred
  • Three to five years of experience in one of the following environments a Long-Term Care, Assisted Living or Community Base Facilities
  • Ability to document resident care plans concisely and clearly. Ability to be patient, sensitive and receptive to both the residents and their family's needs.
  • Must be available for on-Call for weekend concerns or resident admission on a rotation basis.
  • Attend in-services/classes concerning social workers field and to maintain social work certification.
