Job summary

Cedar Grove Rehabilitation & Nursing is seeking a Social Worker to maintain charted progress notes and lead psychosocial assessments and discharge planning for residents and families. You will help address personal, financial and environmental challenges impacting care and collaborate with a multidisciplinary team.

Responsibilities include admission/referral support, crisis intervention, and ensuring comprehensive planning for transition from hospital to post‑acute care.

Qualifications

• Masters in Social work is required.
• Long Term Care Experience is required.
• Experience in Social Services: 1 year preferred.

Responsibilities

Job description

The Social Worker position will maintain documented progress notes of all interactions and planning of care in the resident’s chart. Additionally, the Social Services Director will provide psycho social assessments, intervention, and/or treatment and discharge planning to residents and their families to assist them in coping with their hospitalization, illness, diagnosis and/or life situation and meet the continuing needs post-hospitalization. The Social Worker position will assist residents and families to resolve personal, financial and/or environmental difficulties which interfere with the care management process as well as provide crisis intervention as needed.

Additional skills and experience candidates should possess are, but not limited to:

• Have a strong understanding of referral sources for varying levels of need such as: durable medical equipment companies, home health companies, elder services, psychiatric services/psychiatric programs and housing;
• Versed in PASSARs, Healthcare Proxies, Guardianships, Minimum Data Sets, MOLST forms and OBRA Assessments, Resident Rights, Durable Power of Attorney;
• Able to meet with residents and their families regarding treatment, after-care planning, and long-term care planning and the establishment these services;
• Able to work with a multidisciplinary team, as well as, autonomously with day-to-day responsibilities;
• Able to assist with admission process and/or referrals;
• Strong written and verbal communication skills
• Experience with Point Click Care
• Masters in Social work is required
• Long Term Care Experience is required
