Job summary

Luther Manor, a Life Plan Community, seeks a Full-Time Social Worker (1st shift) to join our organization. This role supports residents' emotional and psychosocial well-being through assessments, care planning, advocacy, and coordination with families and care teams.

Requirements include a Bachelor's degree in Social Work (Wisconsin social work certification preferred) and 3–5 years in Long-Term Care, Assisted Living, or community-based facilities.

Qualifications

• Bachelor's degree in Social Work or related behavioral science
• Wisconsin Social Work certificate preferred
• 3–5 years of experience in Long-Term Care, Assisted Living, or community-based facilities
• Ability to document resident care plans concisely and clearly
• Must be available for on-call for weekend concerns or resident admission on a rotation basis
• Attend in-services/classes concerning social workers field and to maintain social work certification.

Responsibilities

• Assesses needs/goals to maintain or enhance residents' psychosocial, mental and emotional well-being.
• Identify residents’ psychosocial, mental and emotional needs and facilitate access to services.
• Plan discharge when appropriate; arrange home care services and transfers as needed.
• Act as liaison between resident, family and interdisciplinary team.
• Document all resident care plan information and follow state and federal codes.
• Advocate for residents and participate in solutions to identified problems.
• Collaborate with other departments to evaluate residents’ psychosocial wellbeing, mood, and behavior concerns.
• Refer residents and/or families to community resources for concerns in finance, housing and other services.
• Take leadership in investigating resident/family concerns and reporting to authorities as necessary.
• Perform admission, condition-change, and annual assessments to identify and support individual needs.
• Create, review, and update plans of care to support individual needs.
• Promote actions by staff that maintain dignity and individuality of residents.

Education

Summary:

Social worker supportsresidents' emotional, psychosocial, and mental well-being through assessments, care planning, advocacy, resource coordination, and collaboration with residents, families, and care teams to enhance quality of life and ensure person-centered care.

Responsibilities

• Assesses needs/goals to maintain or enhance the psychosocial, mental and emotional needs of our residents.
• Identifying residents’ psychosocial, mental and emotional needs along with providing, and/or aiding in the access of services to meet those needs.
• When appropriate, work with resident, family and other team members to plan discharge. Arrange home care services; assist with transfer to another location.
• Act as a liaison between resident, family and interdisciplinary team.
• Document all resident care plan information and follow State and Federal codes.
• Advocate for residents and participate in solutions to identified problems.
• Collaborate with other departments to evaluate residents’ psychosocial wellbeing, mood, and behavior concerns.
• When necessary, refer residents and/or families to community resources to assist with concerns in areas of finance, housing and additional services.
• Take leadership role in investigation of resident and/or family concerns, follow through to conflict resolution and reporting to state and other authorities as necessary.
• Perform assessment of residents at admission, upon change in condition and/or annually to identify and seek ways to support residents’ individual needs.
• Create, review, and update plan of care to support residents’ individual needs.
• Promoting actions by staff that maintain or enhance each resident’s dignity in full recognition of each resident’s individuality.

Requirements

• Bachelor's degree in Social Work or other related behavioral science. Social Work certificate for state of Wisconsin, preferred
• Three to five years of experience in one of the following environments a Long-Term Care, Assisted Living or Community Base Facilities
• Ability to document resident care plans concisely and clearly. Ability to be patient, sensitive and receptive to both the residents and their family’s needs.
• Must be available for on-Call for weekend concerns or resident admission on a rotation basis.
• Attend in-services/classes concerning social workers field and to maintain social work certification.
