Job summary

MediLodge of Montrose seeks a Social Worker to provide psychosocial support to residents and families in a long-term care setting. You will assess needs, develop care plans, and coordinate services to promote well-being and smooth transitions.

The role requires licensure as applicable, with 1 year in long-term care, and the ability to collaborate across disciplines to support residents and families through challenging times.

Qualifications

• BSSW or MSSW preferred; licensing as required by the state of employment.
• One year of experience in a long-term care environment.
• Ability to maintain confidentiality and provide psychosocial support.

Responsibilities

• Provide direct psychosocial intervention.
• Perform resident assessments at admission, on condition changes, and annually.
• Create, review, and update care plans and progress notes.
• Coordinate resident visits with outside services (dental, optical, etc.).
• Attend and document resident council meetings.
• Assist families in coping with placement, illness, and grief.
• Plan discharge with patient, family, and team members.
• Educate staff on psychosocial issues and patient rights through in-service programs.
• Supervise and guide Social Services Assistants.
• Perform other tasks as assigned.
