Requires a Bachelor’s in Social Work and WI Social Worker license, with 1 year hospital experience preferred. 40 hours weekly, weekday business hours, on-site at the La Crosse main campus, with a strong teamwork culture and growth opportunities.

Qualifications

Responsibilities

• Provide discharge planning, care coordination, and care navigation for acute care patients.
