Job summary

Gundersen Health System is now hiring a full-time Behavioral Health Social Worker in Onalaska, WI at Emplify Health by Gundersen Health System. Social Workers provide assessment, planning, education and referral services to patients and families, and act as consultants and leaders in comprehensive care across the system.

What you will work: 1.0 FTE = 80 hours every 2 weeks, Monday–Friday, core business hours. No weekends or holidays.

Qualifications

• Bachelor’s degree in Social Work or related field required.
• Active WI Social Worker license prior to start date.
• At least 1 year of direct patient contact experience.
• Interviewing and supportive counseling experience.
• Crisis intervention experience.

Responsibilities

• Assess incoming referrals.
• Schedule appointments.
• Coordinate patient care.
• Provide or assist with crisis intervention.
• Meet with patients.
• Facilitate or co facilitate groups.
• Assist with programming needs.

Social Workers provide assessment, planning, consulting, education and referral services to Emplify Health patients and family members as well as act as consultants and leaders in comprehensive care throughout the system, while supporting organizational strategies and goals.

What you will work: 1.0 FTE = 80 hours every 2 weeks Monday – Friday, core business hours. No weekends or holidays. Location: Onalaska, WI

• Assess incoming referrals
• Schedule appointments
• Coordinate patient care
