Hospice of Central PA is seeking a Hospital Based Social Worker in Harrisburg to join the Admissions Department. This full-time role offers 80 hours per biweekly pay period and requires MSW; LSW or LCSW licensure must be obtained within 2 years of hire. The position emphasizes collaboration across hospitals and home care settings.

You will conduct assessments, plan patient-centered care, and support transitions of care with families and care teams. A comprehensive benefits package is included.

• Performs joint home visits with admissions nurses when psychosocial complexity is present.
• Conducts informational and follow-up visits for patients in hospitals, facilities, and home settings.
• Supports initial response for urgent referrals needing immediate response.

Days

Every 5th-6th weekend, 1-2 holidays per year

Essential Functions:

Collaborates with hospice interdisciplinary team, hospitals, and care facilities to develop comfort-focused plans of care,complete comprehensive assessments, and evaluate ongoing interventions.

Collaborates with patients, families, and case managers during hospital discharge planning to ensure safe and appropriate transitions of care.

Conducts joint home visits with admissions nurses when psychosocial complexity is present (e.g., patients living alone, lacking caregivers, or experiencing mental health/substance use concerns).

Performs informational and follow up visits for patients in hospitals, facilities, and home settings.

Supports initial response for urgent referrals needing immediate response.

Qualifications:

MSW required

LSW or LCSW is also required or must be obtained within 2 years of hire date.

Two years’ experience in the health care setting preferred.
