Job summary

Harbor Healthcare System in Dallas is seeking a part-time Social Worker to provide social service activities for patients and their families, coordinating care with the treatment team.

The role involves assessing emotional and social factors, assisting in discharge planning, and documenting services in the patient chart. You will participate in discharge planning and in-services programs to meet patient needs.

Qualifications

• Licensed Social Worker in the State of Texas is required.
• At least 1 year of licensed experience.
• Experience with Hospice or Home Health is preferred.
• Strong written and verbal communication.
• Clean background, drug screen, and driving record.

Responsibilities

• Provide social service activities related to patients and families and coordinate with the treatment team.
• Assess patients' and families' emotional and social factors to estimate capacity and needs.
• Assist in discharge planning and submit written summaries of services provided.
• Assess current status for chart retention and participate in in-services programs related to patient needs.

Job Description

Providing social service activity as it relates to the patients and their families. Thisincludestreatment, planning, and consultation with other staff members. Experiencein assessing the patient’s and their family’s emotional and social factors in orderto estimate their capacity and potential. Assisting in discharge planning from agency service when care is no longer appropriate by submitting a written summary of services provided. Thisincludesan assessment of the patient’s current statusto be retained in the patient’s chart. Participate in discharge planning, and in-services programs related to the needs of the patient.
