Job summary

Community Healthcare of Texas is seeking a Social Worker to provide therapeutic and emotional support to patients and their families within our hospice and palliative care programs. You will assess psychosocial needs, educate families, advocate for patient wishes, document visits, and connect families with community resources.

A Master’s degree in Social Work and reliable transportation are required; hospice experience is preferred.

• Hospice or healthcare experience preferred
• Proficient skills using a computer are required

Responsibilities

• Assess the psychosocial needs of the patient and family members
• Provide therapeutic counseling and emotional support to patients and family members, and education on end-of-life issues
• Contribute to and execute the Plan of Care with the family/patient
• Record daily documentation of visits and contact in accordance with Policy
• Provides referrals to community resources
• Performs other duties as assigned

Community Healthcare of Texas is currently recruiting for a Social Worker.

POSITION SUMMARY

To provide therapeutic/emotional support, education, and resource information to patients and their families; Advocate for patients’ needs/wishes with team members, family members, caregivers, and other providers.

ESSENTIAL FUNCTIONS

• Assess the psychosocial needs of the patient and family members


• Provide therapeutic counseling and emotional support to patients and family members, and education on end-of-life issues


• Contribute to and execute the Plan of Care with the family/patient


• Record daily documentation of visits and contact in accordance with Policy


• Provides referrals to community resources


• Performs other duties as assigned

POSITION QUALIFICATIONS
