Job summary

Heart to Heart Hospice is seeking a compassionate Social Worker to support patients and families at end of life in the Houston area. You will assess psychosocial needs, support advance directives, and facilitate access to resources with a patient-centered approach.

Qualifications

• Degree in social work from accredited program.
• Current State license as per regulations.
• Driver's license, reliable transportation, and valid auto liability insurance.
• One year of social work experience in clinical care; hospice experience preferred.

Responsibilities

• Assess hospice patients to identify psychosocial and care needs.
• Provide individual counseling to patients and families and assist with advance directives.
• Coordinate community resources based on patient/family needs.
• Accept assignments within education and competence.
• Provide on-call coverage as needed.

What You Must Have:

• Bachelors or Master’s degree in social work from a State accredited program
• Current State license required in accordance with State regulations
• Current Driver's License, reliable transportation, and provide ongoing valid and current auto liability insurance
• One year of experience as a Social Worker in a clinical care setting required, hospice experience preferred

What You Will Do:

• Assess hospice patient to identify psychosocial, grief, financial, environmental and community needs as evidenced by POC documentation, clinical records, team meeting minutes and community resource referrals.
• Provide individual patient/family counseling, crisis intervention, information and assistance with preparation of advance directives, funeral planning issues and transfer of responsibilities relative to legal and fiscal issues.
• Facilitate community resources based on patient/family needs.
• Accept clinical assignments that are consistent with education and competence to meet the needs of the patients
• Perform on-call responsibilities and provide on-call service to patients/families as assigned
