Qualifications

Responsibilities

• Conduct psychosocial assessments for adult day health care participants.
• Develop and implement individualized care plans with interdisciplinary team.
• Provide counseling, crisis intervention, and emotional support to participants and families.
• Connect clients with community resources and social services.
• Monitor progress and document changes in condition.
• Advocate for participants' rights and dignity; coordinate discharge planning.
• Facilitate family meetings and coordinate transitions as needed.

Requirements:

• Bachelor's or Master's degree in Social Work (BSW or MSW) from an accredited program
• Current Massachusetts social work licensure (LCSW, LICSW, or LSW preferred)
• Experience working with elderly, disabled, or medically complex adult populations
• Strong knowledge of community resources, elder care services, and social service systems
• Excellent communication, documentation, and case management skills
• Compassionate, patient, and culturally sensitive approach to client care
• Ability to work collaboratively within a multidisciplinary healthcare team
