Job summary

Maryland Medical Day Services Llc is seeking a compassionate Licensed Clinical Social Worker (LCSW) to join their Adult Day Care Center team. You will be responsible for conducting psychosocial assessments, developing care plans, providing counseling, and supporting families in a collaborative environment.

The ideal candidate will hold at least a Bachelor’s Degree in Social Work and possess appropriate licensure in Maryland. This is a full-time position focused on supporting older adults and individuals with disabilities.

Qualifications

• Licensed Bachelor Social Worker (LBSW), Licensed Graduate Social Worker (LMSW), or Licensed Certified Social Worker (LCSW) in the State of Maryland.

Responsibilities

• Conduct psychosocial assessments and develop individualized care plans.
• Provide emotional support and counseling to participants and families.
• Coordinate with interdisciplinary team members regarding participant needs.
• Assist with community resources, referrals, and benefits coordination.
• Maintain accurate and timely documentation.
