Qualifications

• Requires BSW/MSW from an accredited program.
• Current MA social work licensure (LCSW/LICSW/LSW) preferred.
• Experience with elderly or disabled populations and care coordination.
• Strong communication, documentation, and case management skills.

Responsibilities

• Conduct psychosocial assessments for adult day health care participants.
• Develop and implement individualized care plans with interdisciplinary team.
• Provide counseling, crisis intervention, and emotional support to participants and families.
• Connect clients with community resources and social services.
• Monitor progress and document changes in condition.
• Advocate for participants' rights and dignity; coordinate discharge planning.
• Facilitate family meetings and coordinate transitions as needed.
