Job summary

Transcend Hospice is seeking a social worker to provide coordinated in-home care for patients across ages, performing psychosocial assessments, counseling and referrals to meet patient and family needs.

The role requires a BSW or MSW from an accredited program, Texas certification, and a valid Texas driver’s license, with at least one year of healthcare social work experience. Travel to patient homes is expected.

Qualifications

• Bachelor’s or Master’s degree in Social Work from a state accredited program.
• If Bachelor’s degree only, must be supervised by a Master’s of Social Work (§418.114) for hires after 12/02/08.
• Texas Certification; Texas Driver’s License

Responsibilities

• Assess hospice patient to identify psychosocial, financial, environmental, and community needs as evidenced by POC documentation, clinical records, team meeting minutes and community resource referrals.
• Use effective interpersonal relations and communication skills; facilitates the use of these skills by other team members to achieve desirable outcomes.
• Meet mandatory continuing education requirements of the agency/licensing board.
• Demonstrate commitment, professional growth and competency by maintaining a working knowledge of public and private eligibility standards and requirements.
• Promote Agency philosophy and administrative policies to ensure quality of care.
• Attend interdisciplinary team meeting, participate in the patient care planning process and collaborates with Interdisciplinary Team to promote coordination of patient care.
• Accept clinical assignments that are consistent with the education and competence to meet the needs of the patients.

Job Qualifications

Education: Bachelors or Master’s Degree in Social Work from a state accredited program. If has a Bachelor’s degree and is hired after 12/02/08, must be supervised by a Master’s of Social Work (§418.114).

Experience: One (1) year social worker experience in a healthcare agency.

Transportation: Reliable transportation. Valid auto liability insurance.

Environmental and Working Conditions

Works in patient homes in various conditions. Occasional exposure to infectious diseases; ability to travel locally; some exposure to unpleasant weather.

Physical and Mental Effort

Prolonged standing, walking, and sitting required; requires working under some stressful conditions to meet deadlines and patient/family needs; and to make quick decisions and resource acquisition; meet family/patient psychosocial needs.
