Job summary

AdviniaCare Scallop Shell in Peace Dale, RI, is seeking a part-time Social Worker to support residents under the Director of Social Services. You will participate in care plan meetings, assist with assessments and the MDS process, and help residents and families with coping and navigating the healthcare system.

The role focuses on providing compassionate, person-centered care and collaborating with a multidisciplinary team to meet residents’ needs.

Qualifications

• ASW or equivalent degree required.
• Experience with care plans and the MDS process is a plus.

Responsibilities

• Provide social and emotional support to residents, helping address physical, emotional, and spiritual needs.
• Assist with resident grievances and resolve issues promptly and professionally.
• Work with a multi-disciplinary team to develop and implement individualized care plans.
• Participate in care plan meetings and contribute to plans reflecting current research and best practices.
• Assist with the MDS process to ensure appropriate care and services.
• Educate and support residents and families on coping with loss, chronic illness, and navigating the healthcare system.

Key Responsibilities:

• Work closely with our multi-disciplinary team to develop and implement individualized care plans that meet the unique needs of each resident




• Participate in care plan meetings and contribute to the development of care plans that reflect the latest research and best practices




• Assist with the MDS process, ensuring that our residents receive the appropriate level of care and services




• Provide education and support to residents and their families on topics such as coping with loss, managing chronic illness, and navigating the healthcare system

Requirements:

• Associate's degree in Social Work (ASW)




• Part-time position with flexible scheduling to accommodate your needs (24 hours/week)
