Job summary

AdviniaCare Salem seeks a Director of Social Services, licensed social worker (LSW/LCSW/LICSW) to lead our interdisciplinary team. You will ensure holistic, person-centered care for our residents and oversee social assessments, admissions, and family communications.

You’ll collaborate with nursing, therapy, and administration to create a true community of care, mentor staff, and guide transitions for residents and families in a fast-paced senior living environment.

Qualifications

• A degree from an accredited school of social work.
• Licensure in the state of practice (LSW, LCSW, or LICSW) required.
• 2+ years of experience in long-term care preferred.

Responsibilities

• Be a key player in ensuring holistic, person-centered care for residents.
• Lead efforts addressing psychological, social, and physical needs of residents and families.
• Collaborate with nursing, therapy, and administrative teams to create a true community of care.
• Complete social assessments and gather information at admission.
• Maintain ongoing communication and support with residents' families.
• Assist Admissions Coordinator by giving welcoming tours to prospective residents/families.
• Complete pre-admission assessments to help residents transition smoothly into the facility.
