Job summary

Complete Care is a skilled nursing community seeking a Social Worker to support residents and their families. You'll collaborate with a team that values compassion, respect, and excellence.

As part of our Social Services team, you will identify psychosocial needs, document Social History and assessments, and work with an interdisciplinary group to protect residents' rights. We offer a full-time day shift with benefits.

Qualifications

• Must have LSW or CSW.
• Long-term care experience preferred.
• Ability to build and maintain rapport with residents and peers.
• Ability to work independently or as part of a group.
• Computer knowledge.

Responsibilities

• Identify residents' psychosocial, mental, and emotional needs.
• Complete Social History and Psychosocial assessment of residents.
• Collaborate with interdisciplinary team to promote and protect residents' rights.
• Prevent and address resident abuse as mandated by law and licensure.
• Other duties as assigned by Director of Social Services and Administrator.
