Complete Care at Germantown is seeking a compassionate Social Worker for a part-time 8A-12P shift to support residents and families in a long-term care setting. You will assess psychosocial needs, document histories, and contribute to care planning with the interdisciplinary team.

The ideal candidate holds a BSW or MSW and an active Wisconsin license, with at least one year of healthcare social work experience, preferably in LTC/SNF, and strong communication skills to advocate for residents'

Formación

  • Bachelor's in Social Work (BSW) or Master's in Social Work (MSW) required.
  • Active, unencumbered Wisconsin social work license (CSW/APSW/ISW/LCSW or equivalent).
  • Minimum 1 year of supervised social work experience in healthcare; LTC/SNF preferred.

Responsabilidades

  • Assess residents' psychosocial, mental, and emotional needs through visits and observation.
  • Complete Social History and psychosocial assessments upon admission and regularly thereafter.
  • Integrate findings into interdisciplinary care planning (MDS) for holistic care.
  • Advocate for residents' rights, dignity, and self-determination.
  • Address abuse, neglect, and exploitation per law and licensure.
  • Coordinate discharge planning and connect residents with post-discharge resources.
  • Provide emotional support and counseling to residents and families.

Job Summary & Responsibilities: Psychosocial Care & Advocacy

As the Social Worker, you will be the resident's primary advocate and emotional support liaison. Your core responsibilities include:

  • Assessment: Help to accurately identify resident’s psychosocial, mental, and emotional needs through regular visitation and observation.

  • Documentation: Complete Social History and Psychosocial assessment of residents promptly upon admission and routinely thereafter, ensuring compliance with federal and state regulations.

  • Care Planning: Integrate assessment findings into the interdisciplinary care planning process (MDS), contributing to a holistic plan that addresses the resident's unique needs and preferences.

  • Advocacy: Work closely with the interdisciplinary team to promote and protect resident’s rights, dignity, and self-determination throughout their stay.

  • Protection: Prevent and address resident abuse, neglect, and exploitation as mandated by law and professional licensure.

  • Discharge Planning: Coordinate discharge planning services, ensuring smooth transitions for residents back to the community or to other levels of care, and linking them with necessary post-discharge resources.

  • Support: Provide emotional support and counseling to residents and families coping with illness, disability, and long-term care placement.

  • Other: Duties as assigned by the Administrator.

Qualifications

We are looking for a dedicated professional who is licensed and passionate about long-term care.

  • Education: Bachelor's Degree in Social Work (BSW) or Master's Degree in Social Work (MSW) required (minimum of a Bachelor's degree in Social Work).

  • Licensure: Must possess an active, unencumbered (CSW Certified Social Worker, APSW - Advanced Practice Social Worker, ISW - Independent Social Worker, LCSW - Licensed Clinical Social Worker), or equivalent state social work license in the state of Wisconsin.

  • Experience: Minimum of one (1) year of supervised social work experience in a health care setting working directly with individuals. Experience in a Long-Term Care (LTC) or Skilled Nursing Facility (SNF) is highly preferred.

  • Skills & Competencies:

    • Demonstrated ability to work independently while functioning as a collaborative member of the interdisciplinary team.

    • Proficiency in computer usage for documentation and Electronic Health Records (EHR/MDS).

    • Exceptional ability to build and maintain rapport with residents, family members, and peers.
