Job summary

Complete Care at Bayshore is seeking a Social Worker to support residents and their families with psychosocial care as part of our extended team.

You will complete social histories and psychosocial assessments, working with an interdisciplinary team to promote residents' rights and protect them from abuse.

Qualifications

• Identify residents' psychosocial, mental, and emotional needs.
• Complete Social History and Psychosocial assessments.
• Collaborate with interdisciplinary team to protect residents' rights.
• Prevent and address resident abuse as mandated by law.
• Must have LSW or CSW.
• Prefer prior long-term care experience.

Responsibilities
